Basic Package Advisory Committee advice on the Transcatheter Aortic Valve Implantation in patients with symptomatic severe aortic valve stenosis in combination with high surgical risk

The Basic Package Advisory Committee (Adviescommissie Pakket, ACP) of the Dutch Health Care Institute (ZIN) issues advice to the Board of Directors of the Dutch Healthcare Institute. These recommendations relate to proposed reports and assessments in which the Dutch Healthcare Institute advises the Minister of Health, Welfare and Sport on the proposed policy concerning the basic package or on developments that could lead to changes in the basic package.

During the meeting on August 28, 2020, The Basic Package Advisory Committee (ACP) discussed the inclusion of TAVI in the basic insurance in patients with symptomatic severe aortic valve stenosis in combination with a high risk of surgery. It is the first time when medical specialist treatment (using a medical device) is discussed by the Committee.

The Committee's point is that the use of TAVI in patients with symptomatic severe aortic valve stenosis in combination with a high surgical risk complies with the ‘state of the art and science’ and is therefore effective. The Committee supports the cautious and careful approach of the ZIN regarding expansion to other subgroups due to the lack of long-term data. The Committee is also positive about the constructive attitude in making appropriate agreements for use and the willingness to collect and evaluate data. The Committee supports the proposal of the ZIN to work with parties to see how cost-effectiveness can be improved.
